Status report on ACE and angiotensin type 1 receptor blockade
- The role of ARBs as an alternative to ACE inhibitors or as an additional means of treatment of patients with heart failure has been evaluated in three recent trials: ELITE (Evaluation of Losartan In The Elderly), RESOLVD (Randomize Evaluation of Strategies for Left Ventricular Dysfunction), and ELITE II, the Losartan Heart Failure Survival Study, which was a follow-up to the first ELITE study.
- ELITE compared captopril (50 mg 3 times/day) and losartan (50 mg/day) in 722 elderly patients with systolic dysfunction.87
- The RESOLVD pilot study (N = 768) compared enalapril (20 mg/day, N = 109) candesartan (4, 8, or 16 mg/day, N = 327) or candesartan plus enalapril (4 or 8 mg plus 20 mg/per day, N = 332) in patients with NYHA class II to class IV heart failure.88
- ELITE II was a larger trial in 3152 patients with NYHA class II to IV heart failure. Patients were given captopril (50 mg 3 times/day, N = 1574) or losartan (50 mg/day, N = 1578).89
