III: Clinical Imperatives When Treating Patients with Diabetes

  • Criteria for the diagnosis of diabetes include:

    - Symptoms of diabetes plus a casual plasma glucose concentration >200 mg/dL.

    - Or an FPG >126 mg/dL (following no caloric intake for >8 hours).

    -Or a 2-hour postload glucose >200 mg/dL during an OGTT. (The OGTT is not recommended for routine clinical use.)

  • IFG and IGT identify an intermediate group of persons whose glucose levels, although not meeting the criteria for diabetes, are nevertheless too high to be considered normal.

  • ADA criteria for IFG are somewhat lower than those of the American Association of Clinical Endocrinologists (AACE) and the American College of Endocrinology.

  • Patients with IFG and/or IGT are now referred to as having "prediabetes," indicating that they are at relatively high risk for development of diabetes.
