COURAGE: Defining optimal care
- In patients with stable CAD it remains unclear whether an initial management strategy of intensive medical therapy, a reduction in risk factors, and lifestyle intervention (known as optimal medical therapy) plus PCI is superior to optimal medical therapy alone in improving disease progression and reducing the risk of CV events.
- Approximately 85% of all PCI procedures are performed electively in patients with stable CAD, even though evidence has shown that PCI only decreases angina frequency and improves short-term exercise performance.
- The COURAGE trial investigated whether PCI together with optimal medical therapy improves the long-term prognosis of patients with stable CAD by reducing the risk of death and nonfatal MI compared with optimal therapy alone. (1)
